Coverage Details :

Mode of payment/reimbursement

Reimbursement of medical expenses is to be made through account payee cheque to the covered victim against both IPD and OPD benefits availedDirect payment facility is to be provided to the covered victim for IPD benefit availed One time payment through Pay Order is to be made to the nominee of the covered deceased victim in case of accidental death

1. Accidental Death Benefit (ADB): If an covered member dies directly from an accident caused through external and violent means (except death due to any unlawful activities
2. Temporary or Permanent Disability (PTD) due to Accident: In case of Temporary or Permanent Disability resulting from bodily injury directly through accidental means,
   1. Permanent disablement
   2. Temporary total disablement
   3. Temporary partial disablement
3. Hospitalization (IPD) due to Accident
   1. Types of Accidental Hospitalization 
      1. Head Injuries
      2. Chest Injuries
      3. Fractures, Dislocation, Scrapes and Cuts Fracture
      4. Soft Tissue Injuries
      5. Any other injuries suffered as a result to accident which requires hospital admission for treatment on recommendation of a registered doctor

Exclusion Policy

1. Death caused by self-inflicted injury or the commission of or attempted commission of an assault or any unlawful act, or being engaged in any illegal activity or felony.
2. Participating in illegal activity is not covered.
3. Suicide while sane or insane. 
4. The condition of Acquired Immune Deficiency Syndrome (AIDS), or any AIDS related illness or HIV virus

How to claim

1. The claimant (Rider or Passenger) should immediately notify to Pathao regarding the incident
2. When to claim
   1. within 45 working days of occurrence in case of loss of life; 
   2. within 15 working days of occurrence in case of admission to a hospital or treatment center for IPD treatment; 
   3. 10 working days of occurrence in case of OPD treatment; to avail the benefit as covered under the agreement.
3. Member/Beneficiary shall submit all Documents in relation to the Claim to the Pathao within ten (10) working days from notifying/reporting the claim.
4. Upon receipt of all supporting documents, GDIC shall settle the Claim within 10 (ten) working days calculated from the date of document submission.

Required Documents

1. Secured Member’s National ID Copies/Birth Registration certificate /Passport copy.
2.  In case of Permanent and Total Disability due to Accident, a proper doctor/ orthopaedics’ certificate mentioning the reason(s) of Permanent and Total Disability along with supporting all medical reports such as X-ray, City scan, MRI or such relevant documents (if any).  
3. In case of Hospitalization due to Accident, a proper doctor/ orthopaedics’ certificate mentioning the reason(s) of Hospitalization along with supporting all medical reports such as X-ray, City scan, MRI or such relevant documents (if any). Also need to provide a photocopy of the Doctor’s prescription/ advice copy for hospitalization and discharge certificate from the Hospital.
4. In case of death, Death certificate from the Municipal Body / Attending Physician / Hospital Certificate from the concerned authority. 
5. In case of Accidental death, Death Certificate from the Municipal Body / Attending Physician / Hospital Certificate from the concerned authority along with Post-mortem report / Police Certificate

Eligible Hospital List

1. Users can claim indirect payment from any hospitals in Bangladesh
2. These Hospitals from where beneficiaries can avail direct payment facilities for Inpatient Department (IPD) treatment.
